Form 8453, does it really work for stocks, options

Form 8453.....so I understand correctly
As long as the substitute document is formated like the Sch D-1 (like a spreadsheet) this document does NOT need to accompany the tax return but the document NEEDS to be in the file. Netted out ST gains/losses and LT gains/losses need to be entered into Sch D. Then the taxpayer can efile his tax return w/o refund delays if applicable?
