I have a client who was professor at the University of Miami. She gave me a W-2 today that I have never seen before. Top of the form looks like the ones we all see and has all the Fed info. It then says, "University of Miami 2006 W-2 Wage and Tax Statement". The bottom part has no space for state information, only these descriptions:
Box C: Employer name (U of M)
Box D: Check Distribution (empty)
Box E: Job (this has a code "B" in it)
Box F &G: Employee's info
Can one of you great Floridians explain this to me, especially box E?
