    Direct Debit - IA

    Applying for the IA I entered the direct debit account information. IRS messed things up and agent told me to mail in the direct debit info again. I was trying to find a form for this but could not.

    Other options would be to write a letter or use IA form again, which I am hesitant to do unless I put in only account info and personal info. Still IRS might get confused more. What would you do?

      Actually I'm dealing with the exact same thing now. It seems that form 433-D is what you can use. At least you can look at that form.
      You have the right to remain silent. Anything you say will be misquoted, then used against you.

          Actually, I did and dismissed. If nothing else comes up I will give it another try.
