Does anyone have a template or NV LLC Dissolution form

Client sold the one asset in the name of the LLC, we need to inform NV that the LLC has dissolved. I assume this requires some Corporate minutes and a form of some kind. Where can I find a sample?

One page, requiring only the name of the LLC being dissolved, and $75? That's it?

Why would people pay $1000 for a 1 hr expedited service, or $500 for 2 hr expedited service, or $25 - $125 for 24 hour service? I don't get it.

I'm surprised they don't ask for a corporate resolution of dissolution, or anything else. My client is paying a monthly fee to a registered agent to maintain this LLC in NV. I'm hoping this will eliminate the need for this payment.
