no depreciation but has 4 vechiles used - how to list

What to do when mileage is to go on the back of schedule c but have 4 vechiles and no depreciation so the 2nd page of 4562 is not to be used? Also another question - what if client has 4 vechiles all used more than 50% and the description section only has room for 3?

My tax software, will generate an additional statement, and on page 2 of 4562 or appropriate form will print "see statement".
