    Third Party Release Form

    Does anyone have a good example of a third party release form. Customer is wanting me to discuss his taxes and financial situation with a financial advisor. I need something that says it is okay for me to talk to him.

    I would write it up in plain English.

    Plain English documents hold up better than something with a bunch of fine print that was written by attorneys.

    Just write up a simple authorization that contains your client's name and address, the person he wants to you talk to, and their name and address, and the year or years of tax returns... oh, and your name and address, too LOL

        Also, the advisor may have authorization forms they use; maybe even already signed by the client. Have gotten copies of these with people's stuff this year.

          I did not think of that. I made up a really simple one and emailed it to my customer. He is suppose to come by but just in case I went ahead and emailed it. I'll also find out if the advisor has one.
