i went to the

Client is a minister and I have to send in a Form 8453. Question is, where do I send it?? IRS.gov lists only Forms 8453-F & 8453-P with an Ogden mailing address. Or would I send it to where the Form 1040 would have went had I mailed it?? Advise please.
