    Form 5754 (Multiple Gambling Winners)

    Who out there is an expert on this form?

    Can this form be used for slot machine play?

    If so when must the Form 5754 be presented to the Casino?

    Is it the Casino's responsibility to furnish the Form 5754 or must the player have it in his/her posession prior to pulling the slot machine handle?

    Does anyone have any additional information other than the IRS instructions that might help cleafify how and when it is used?

    I am not an expert in the Form

    but I looked it up on the IRS website and I read the form and instructions. It seems pretty straightforward. You file the form with the IRS as part of your tax return if a casino or lottery commission treats you as the winner of a wager and you transfer some of the winnings to someone else. For example it is common for groups of people to share among themselves one or more lottery tickets. But in my opinion taking a copy to the casino would be a jinx.
