An employee of one of my clients brought this in to my clients' bookkeeper demanding no withholding.
It looks like an official IRS form, however, the last form revision is dated November 1998.
Further, the guy signs it and appends his signature with 'UCC1-207 UCC1-1026 without prejudice'. (Plus uses the 't' in his name to effectively cross out the signature).
I've advised the bookkeeper that if the employee wants no withholding he should complete & sign a current W-4.
In the meantime, I was wondering if anyone had heard of this and could give some background/history on when it was last used (if any) as well as any theories on what this scam may be.
