Where can I find instructions for a city tax return? I've never done one and want to make sure I do it correctly. My software program list a bunch of jurisdiction codes but they do not define which code is for which city. My client used to live in Long Pond, PA (zip code 18334) before moving to Michigan.

City Tax Return

Try Googling. Go to Google and type in: City Income tax forms for Long Pond PA. Then click on whatever you need that comes up.

Find the municipality and/or school district the taxpayer lived in; that's the township, borough or city (not the town for zip code purposes, ie Long Pond.) I'm pretty sure it is Tunkhannock Twp/Pocono Mountain SD. The collector is Berkheimer Assoc. 800-360-8989. Go to www.hab-inc.com for forms download. Rate is 1%. Post if you need more info.
