Is there a form number for a substitute W-2 ????

I believe the form is 4852.

My software does prepare the State portion on a Fed prepared form, the 4852, but I don't know if this would suffice for filing. It should.

What do you think?

That one doesn't bother with state info, so you will need something else as well.

Form 4852, line 7g for state info, and there's even a line 7h for local info.
