IRS Inputting Incorrect Ln 12 Tax Liability on Form 941?

Has anyone else seen IRS notices for Notice of Tax Due in which the IRS somehow has a mystery amount for the liability? I'm dealing with 4 of them right now, but I believe the firm has more overall. When we pull transcripts, the amount the IRS claims is the tax due shows as liability per taxpayer. In other words, the "liability per taxpayer" on the account transcript is the same amount on the notice, but does not match the Ln 12 liability reported on the Form 941 prepared by our firm and submitted by the taxpayer. *Note - our firm does still do all 941s on paper using a software. We do not pay for e-filing.

If anyone else has dealt with these, how did you resolve them? I am so very, very weary of the headache trying to get through to the IRS. I actually never minded calling the IRS until the plandemic. Now I'd rather have drinks with Bill Cosby in HH Holmes's hotel bar.
