    form 8889 HSA

    Haven't had to deal much with HSA's with my clientele but now have an issue with a 66 year old person.
    He still works and his 2018 W-2 shows $4,450 (W) in box 12 but he enrolled in medicare in May - so he became ineligible then.
    I'm trying to figure out how he gets the disallowed contribution amount onto his 2018 return by using the worksheet on form 8889.
    The limitation worksheet shows $17,800 total and $1,483 limitation.
    I assume this means he'll report the diff. from $4,450 as 2018 income but I can't seem to see where this is going on the 1040.
    Have entered these amounts on the 8889 :
    Line # 2 $ 4,450
    Line # 3 $ 1,483
    Line # 5 $ 1,483
    Line # 8 $ 1,483
    Line # 9 $ 4,450
    Line #11 $ 4,450
    He also had equal amounts of $ 4,411 for lines 14-a and line 15. I have $ 2,967 on line 14-b
    I must be doing something wrong. Anyone have a comment on this ?

    You treat the excess contribution the same as an IRA
