    Religious exemption

    Does any one know if IRS keeps a copy of the Form 4361 Religious exemption? I need to see if I can get a copy of one filed 20 years ago! Or, can taxpayer apply again?

    It may be possible. Copy of letter received 2/1/2011 with an approved 4361 gives address of: IRS, Ministerial Waivers Unit, PO Box 16325, Philadelphia PA 19114-0425. PH: 267-944-4034. It advises TP to "retain copy of approved 4361 for your permanent record." TP can contact them and ask them to search their records for it. Also, the Social Security Admin office may have it on file. Does he have a copy of the application?
