940 Amended

Hard as it may seem, a client messed up their 940-EZ and now it has to be amended. Anyone know the procedure? Is it just redoing it and writing "Amended" acros the top, or s/t else.

See the 940EZ instructions. There is an "amended return" checkbox above part I. Show the figures that should have been on the original return and attach an explanation.
