couple, mjf, wife applies to the market place for insurance, receives a 8962 for her. Husband's name is on th e form, but hers on top, and Part II: Coverage Household, just hers. But when I add the form from my software, husband's name and SS# come up instead of hers and I can't seem to change it. 8962 does say: name shown on return. Does it have to have her name on top, or does it matter?

Who is the owner or spouse does not appear on the 8962, but you are likely seeing that on an input form in your software. If you post what software you use, someone can probably help as to how to get it to show correctly.
I think you are saying that on the form 1095A only one person is showing as being covered in part II? Do the premium numbers in the month details also look to be for only one person?

The IRS instructions specifically say to use the first name/SSN.
Name. Print or type your name exactly as you entered it on your tax return. If you are married and filing a joint return, enter the first name that appears on your return.
Social security number. The social security number on this form should match the social security number on your tax return. If you are married and filing a joint return, enter the first social security number that appears on your tax return.

One way to think of it is as a single occurrence form (only one allowed per 1040) the form belongs to the tax return, not the individual. For example Schedule SE would list who it was for. You could have one for taxpayer and another for spouse - more than one per return. Schedule A you can only have one, and it lists the taxpayer's SSN (first SSN on return).
