      It doesn't ask for date of birth on the form. I would say yes you can.

          Is DOB ever necessary for Form 4868?

          Interesting question.

          I've filed numerous Forms 4868 (with/without a payment via bank debit) and I've never had to provide any DOBs. Perhaps the information was lurking in the client data base, but I seriously doubt if it was an issue at all.

          OTOH, I *DO* have DOBs for all clients and their dependents.
