Has anyone seen a "recommended" or sample Form 3115 that would include appropriate language to be filed in the form? It would be nice not to have to reinvent the wheel especially when many of us will be filing these.

I agree that the NATP webinar was great and the text and example Forms 3155 to download are also great.
Lisa Ihm (I've learned a lot from her live courses) has a Form 3115 Worksheet Master available for download free and a brief course/booklet (about 40 pages) on Form 3115, full of examples, for $20 that is worth every penny:
