Revised 1040 Full-year coverage block.

Are these forms available anywhere so we can view (ACA)?

I see on the revised 1040 for 2014, line 61 that this is the block where you check for full coverage. This would cover the bulk of the clients, I would think. Any other entries for full coverage? Is proof required?
