    Signing return as POA

For that nursing home client...I have POA which I used to get IRS transcripts for prior years. I have never signed a return as POA for a client. Do you just sign and put "POA" after signature?

Only if your POA allows the signing of tax returns. (I don't put that on my POAs. I insist that whoever is responsible for the person who cannot sign have a POA that includes signing tax returns.)

I would want a family member to sign if the client is unable to to sign for themselves. The 2848 will allow you to sign (if you check the box) only under very limited conditions.
In other words, a democratic government is the only one in which those who vote for a tax can escape the obligation to pay it.

I agree with the others for liability purposes I don't sign any documents other than "statement of facts provided by TP to me".
Believe nothing you have not personally researched and verified.

The POA should only give you permission to talk to the IRS on the clients behalf. I would NEVER sign a client tax return.
