    .9% Additional Medicare Tax W-2 Reporting

    If you prepare W-2s for business clients, please note that the extra .9% Medicare Tax for those subject to the tax is not shown separately in a separate field on the W-2. It is included in W-2 Box 6 (Medicare Tax Withholding).
    (How the software programs are going to handle the extra amount and include it on the line for FWT is anyone's guess at this point).

    The W-2 instructions note that is how it's reported.
    If you go to irs.gov and in the search field enter "Additional Medicare Tax" it brings you to a FAQ screen dated in November - here's the question that discusses this -
    52. How does an employer report Additional Medicare Tax on Form W-2?
    There is no change to the boxes on Form W-2. An employer will enter the total employee Medicare tax (including any Additional Medicare Tax) withheld on Medicare wages and tips in box 6 (“Medicare tax withheld”). A railroad employer will report Additional Medicare Tax in box 14.

    The 8959 has part V for withholding reconciliation. http://www.irs.gov/pub/irs-dft/f8959--dft.pdf. The amount on line 24 then gets added to other tax withheld on line 62.
