1099 -b future contracts

I rarely come across one of these and would like to see if someone can advise me.
1099- b

Amounts in #9 profit/loss on futures contract
and
amount in #12 aggregate profit /loo from lines 9,10, and 11

those are the only two boxes filed out.

I believe they carry to form 6781, but not sure. Can anyone advise as to what lines they carry to.

Futures Contracts

The amount in box 12 should appear on line 1 of Form 6781

The rest of Form 6781 is rather mechanical

The amount gets split: 40% short-term and 60% long-term

and those amounts are then carried to Schedule D
