Tax Return Routing Sheet

Does anyone have a tax return routing sheet (i.e. sheet that accompanies the source documents and return around the office ) they would be willing to share? I had one that had the clients name on top, showed which returns were to be prepared, had a place for notes and then had some steps that I wanted the preparers to check off having performed. I went to print some new sheets and for some reason I can not find it on my server. I've been wanting to change them any how and figured this might be a good opportunity to do so.

Check your software. ProSeries has a "client status" page. I find it quite sufficient. You can even pdf it and make any additions to it that you need if you have Adobe Acrobat Pro
