Change in Year End

I have a non-profit client with a year end of Sept 30. They wish to change to a calendar year. The only IRS form availabe for this change is IRS 3115 - Application for change in Accounting method.

Am I correct on using this form for a change in year end? If so, the cost is an astronomical $650 ?
