Roth IRA Distibution rolled to Roth

Client withdrew money of $2500 from Roth IRA, 56 days later placed in another Roth IRA. 1099-R shows J (of course). I entered 1099 with J code and marked rollover but do I need to place this on another form to show it was rolled over? I looked at 5329 but did not see anywhere to do it.

Try Part III of 8606, I am not even looking at it, so if that is totally wrong, my bad.
If you loan someone $20 and never see them again, it was probably worth it.

Did it show up on page 1 of 1040?
