My biggest fear

That is my biggest fear to. Even trying to be religious about having only one file open at a time it does not always work out that way.
I have found papers in the wrong file. I also found a letter to a client that I insisted on having mailed (since this is what I wrote on the copy of letter). I felt quite embarrassed.
When I mail form 9325 after e-filing I worry to put mix up the letters with the right envelope. As a matter of fact, I recently received a letter asking for a donation from a local non-profit and the address on the letter was for the power co. While it didn't matter to me, I thought it might matter to the power co. who surely normally donates much more than I do.
