Worksheet for 1099-C

Does anyone know where I can get good worksheets for calculating COD income & gain/loss for Forms 1099-C, Cancellation Of Debt?
I need one for calculating Short Sales also. I need something more than the ones in TTB. My software doesn't include any worksheets for these.
