    Health Savings Account Contributions

    I have a client that enrolled into a HSA through his employer. At the beginning of the year employer contributed $600 pre-tax. Something happened and the contributions started coming out after-tax. Total amount after-tax is $3,500.

    Client did not receive a Form 5498-SA showing their after-tax contributions. He did receive a 1099-SA showing distributions. W-2 shows the $600 the employer gave pre-tax.

    Is it correct that the client should have received a 5498-SA showing his contributions? I want to make sure before I have the client call about it. Another client I had got a 5498-SA showing just employer contributions. TTB says that the taxpayer should receive one.

    5498-sa

    The issuer of the form 5498-sa has until may 31 to provide a copy to the Participant.

      Yep, same as IRA contributions, which can be made until 4-15 for prior year.

      Client could get a copy of account statement if needed.
