Form 990 due date is 15th day of 5th Month (Fiscal) or 5-15 (Calendar)
Is this what you are looking for?
Use Form 8868 to request an automatic 3-month extension of time to file Form 990, 990-EZ, or 990-PF and also to apply for an additional (not automatic) 3-month extension if needed. Do not apply for both the automatic 3-month extension and the additional 3-month extension at the same time. For more information, see Form 8868 and its instructions

Yes
Thanx, Sandy. I was looking for a separate form and overlooking Form 8868. Yes, have a non-profit ending 30 June that was due 15 November that we extended to 15 February but I won't be getting their info until Friday! I may try to complete it, but then it has to make the rounds of directors to review, sign, and mail. And, my 2011 clients are dropping off in masses. Have you had success giving reasons? They did open a second location; perhaps books and records were packed up?

Lion, I have not filed the add'l ext on Form 990's, but we used to use the "generic" on the add'l extensions for 1041, etc before the automatic date changed something to the effect
"Additional Time is Requested to Prepare a materially complete and accurate return" - I do not believe we ever received a denial.

Part 2 of Form 8868 is used to request an additional 3 month extension. You'll need to give a reason. Here are the two most common reasons I've given over the last 12 years:
"Additional time is needed because the organization is waiting for the completion of the annual independent audit of the financial statements."
or
"Additional time is needed to assemble documents and records necessary to file a complete and accurate Form 990."

You have some extra time this year.
I sent a efile in this week and found the IRS is taking a 2 month holiday on 990s to update the system. None will be accepted via efile till 3/1. Those due Jan and Feb received an automatic extension.
Now I just need to remember to resend the return on 3/1 when the system is back up.

Thank You, Everyone
Thank you for very thorough replies. I'm dealing with a head cold/sinus infection and medications, so will not try to complete this 990 (do only one or two of these each year) this weekend. Going to keep the reason short and sweet, and appreciate your examples. They may have had second extensions over the years with their former preparer, as this treasurer is notoriously late. Haven't been efiling these; maybe this is a good time to start.
