    Non-Deductible IRA Transferred to Roth

Had client who made a $5K non-deductible IRA contribution in 2009 and another $5K non-deductible contribution in 2010. Then transferred the $10K to a Roth in 2010. She has a 1099R that was not reported on the 2010 return. What are the corrections that need to be made at the point?

Assuming TP has made only 2 traditional IRA contributions totalling only $10K, then do a 1040X showing the 1099R and an 8606 with this basis amount.
