3 on the page. That is normal with all being identical EXCEPT for 1 that has "taxable amt not determined" box checked and NO amt in box 2a. The corrected box is NOT checked.

Are you saying that you got a 3-up sheet, one copy for federal, one for state, and one for recipient, the account numbers (if any) are identical, but the contents aren't?
I'd ask for a corrected version. In the meantime, can you figure out what it should be? Are there amounts in any of the contribution boxes? Or does the recipient know whether or not there's a basis?
