I believe there is a form that the childcare provider gives to each customer with the amount of money received. Is this an official form or just one that is made up?

Child Care Provider -Form?

No form requirement
So far as I know, there is no requirement for the provider to give the parent anything resembling a "form" showing day care expenses. (This is a separate issue from the Form W10 providing the tax ID for the firm.)
Most providers do so (itemized annual statement or whatever) out of courtesy and/or good business practices.
I'm not sure what, if any, reporting requirements exist for on-site day care that is provided by some employers (and can also alter W2 wages!).

The parent gives the W-10 to the provider. The provider isn't required to give one.

A local firm, of national repute, provides high quality, discounted onsite daycare to its employees who pay a maximum of something in the range of $100/week.
The background documentation the employee receives is almost enough to send them to a professional tax person. (And the documentation is SUPERB!!) In every case that I ever dealt with, what they "paid" has little to do with what shows up on the Form 2441. Their taxable income is frequently "bumped up" by the value of the fringe benefits indirectly received. Sometimes you had to enter on the Form 2441 "See W2" with no dollar amount - some tax software cannot even handle such!
