I do an individual letter for the specific engagement. I bill collections work hourly, paid in advance, with no promise of specific results.
An OIC costs $3000 or more. The first big step is for the client to complete a financial statement--might as well use IRS Forms 433 A & B if you can find them. You can't make a decision about whether to even file an OIC, much less how to structure an offer, until you get the facts. In most cases a different approach is called for -- usually either ignoring the bill or else doing a payment plan.
If the client has legal issues such as bankruptcy potential or non-filing, I include a referral to an attorney in the engagement letter. I shouldn't say "if." That's a pretty standard thing.
