Having trouble with Form 5498-SA
Box 1 has $ 505.00
Box 2 has $1078.00

Any help on, where and how much to report on what form is needed.
Does the amount in box one go on 1040 Line 25?

Per Form instructions, Box 1 is the employers contribution, so taxpayer does not get a deduction for this.

Box 2 is the total contributions to the HSA. So deductible portion would be Box 2 - Box 1. Try filling out the screens in your tax software, that usually does the trick.
