client got a notice from IRS for 2008 saying that $37,000 was added to their income because 1099-int was left off original return. Problem is the 1099-int WAS on the return - it was for $374.29 and IRS says it was $37,429. OOPS!

Oh, I think I can top that one.
I filed a return for my client's dead aunt in 2007. She died in 2007.
The IRS filed it for 2008 and even sent her an economic stimulus check!
It was a paper return, and they told my client that I filed it on the wrong form! They said I filed a 2007 tax return on a 2008 form!
I just about lost my grits over that one.
Now, the IRS has messed this up so badly that I had to send in the original tax return with a letter telling them THEY filed the 2007 return in the 2008 year, that Auntie was graveyard dead in 2008 and unless she is working a streetlight district in Heaven and droppin pennies down here, THEY are wrong.

my problem
with this whole mess is that I have to take my time/effort/expense to CORRECT an IRS error that is so obvious! It should be able to be fixed by phone, but Nooooooooo I have to send them "documentation" - like I already sent it the first time WITH the return you IDIOTS!!!!

Whoever issued the 1099-INT may have incorrectly formatted the 1099-INT when they transmitted it to IRS.
