What I have found
Is the retired teacher through CALSTRS pays the district or Medicare, the Medicare Premium. The district or CALSTRS does not pay the benefit on behalf of the retiree.
If the t/p is paying for the Medicare premium or deducted from their retirement benefit, then yes, it is an Insurance deduction on Schedule A medical.
