    Filing a fiduciary form 56

Retired couple file jointly - a simple, non-business type of return.
Wife now has dementia & has recently been placed in a facility.
In January, the husband obtained legal guardianship for her from the county court.
As I understand it, a fiduciary form 56 needs to be filed to allow husband to sign for her on the 8879 for e-file.
Can form 56 be attached as a PDF to the e-return or must it be filed separately with a form 8453 or other document ?
Also, the form 56 instructions seem to indicate that part three of the form need not be completed in this case.
    Thanks for comments.
