I posted in an earlier thread that, to me question 3 and 5 were kind of redundant and related to each other. The IRS law rep said since we scan all documents and document questions and answers via a questionnaire that answering yes to all of 3 and 5 both is the right approach. The additional lines on 5 deal with obscure documents that a preparer relied on. But that answering yes to 5 doesn't mean you have to list any other documents in the space provided in 5 if those are from line 3. Such obscure documents would be court documents for adopted children, or temp guardianship for relatives etc.. But then again those could have been documents from line 3.
